Measles cases in the United States have already climbed to 2,318 in 2026, surpassing the record total reported during all of last year. According to the Centers for Disease Control and Prevention, the new count reflects confirmed infections reported through July 23, with more than five months remaining on the calendar.
The milestone places the country deeper into its worst measles year in more than three decades. According to the Associated Press, the United States recorded 2,289 cases in 2025, which was already the highest annual total since 1991. The 2026 figure passed that mark before the end of July.
The CDC reports that 2,302 of this year’s confirmed measles cases involved residents across 45 jurisdictions, while 16 involved international visitors. According to the agency, 93 percent of all confirmed infections were associated with outbreaks. That includes 782 infections connected to outbreaks that began in 2026 and 1,371 connected to outbreaks that started in 2025.
Those numbers show that the current surge is not being driven only by isolated travel cases. Per the CDC, the country has documented 35 new outbreaks this year, with an outbreak defined as three or more related infections. The agency also cautions that its weekly total includes only confirmed cases reported by health jurisdictions, even when state officials are aware of additional probable infections.
That distinction matters because researchers and public health officials believe the actual number of measles cases could be much higher. According to the Associated Press, some infected people never seek medical treatment, while others may not receive testing or become fully connected to a public health investigation. Limited surveillance and incomplete household information can create additional gaps in official reporting.
South Carolina offers one of the clearest examples of how quickly the disease can move through a community. According to the South Carolina Department of Public Health, an outbreak centered in the Upstate region produced 997 confirmed cases between October 2025 and March 2026. The department formally declared the outbreak over in April after the state completed the required period without a new case.
State officials acknowledged that even that massive number might not capture every infection. Dr. Brannon Traxler of the South Carolina Department of Public Health told the Associated Press that the undercount was “impossible to know and may vary during the course of an outbreak.”
Measles cases have also remained a serious concern in Utah. According to the Utah Department of Health and Human Services, 514 Utah residents had been diagnosed during 2026 as of the state’s July 21 update. Another 197 Utah residents were diagnosed in 2025, bringing the state’s total across the prolonged response to more than 700. Utah officials reported that about 9.6 percent of confirmed infections were breakthrough cases involving vaccinated people.
According to the Associated Press, researchers using genetic analysis and mathematical models believe the major outbreak near the Utah and Arizona border could be three to four times larger than confirmed reporting suggests. The outlet also reported that researchers examining the 2025 West Texas outbreak believe the real total might have approached twice the 762 confirmed cases.
Rémy Pasco, a mathematical epidemiologist working on the Texas analysis, told the Associated Press that “there’s just not much you can do about measles once you have it” when community vaccination coverage is inadequate.
The expanding count is happening as childhood vaccination coverage continues to fall below the level public health experts say is needed to slow community spread. According to the CDC, MMR vaccination coverage among United States kindergarten students dropped from 95.2 percent during the 2019 to 2020 school year to 92.5 percent during the 2024 to 2025 school year. The agency estimates that approximately 286,000 kindergarten students were left at risk during that latest school year.
The national percentage can also conceal much larger gaps inside individual schools, neighborhoods and communities. Per the CDC, communities generally need vaccination coverage above 95 percent to maintain strong community immunity. When the virus reaches a concentrated population of people who are not protected, measles cases can spread rapidly even when statewide coverage appears comparatively high.
According to the CDC’s measles guidance, the virus spreads through the air when an infected person coughs or sneezes. The virus can remain in a room for up to two hours after that person leaves, and as many as nine out of ten unprotected people in close contact with an infected person may become sick.
The CDC lists high fever, coughing, a runny nose, red or watery eyes and a rash among the most common symptoms. The agency warns that complications can include ear infections, diarrhea, pneumonia, brain swelling and death, with children younger than five, pregnant women, adults older than 20 and people with weakened immune systems facing an increased risk of serious illness.
According to the CDC, one dose of the MMR vaccine is about 93 percent effective at preventing measles, while two doses are about 97 percent effective. The agency identifies vaccination as the best protection against infection and advises people with questions about their vaccination history, possible exposure or upcoming international travel to speak with a health care provider.
The United States declared measles eliminated in 2000, meaning the country had stopped continuous domestic transmission for at least 12 months. According to the CDC’s historical overview, that achievement followed decades of widespread vaccination after the first measles vaccine became available in 1963. Before vaccination, an estimated three million to four million people were infected in the United States each year.
That elimination status is now under review. According to the Pan American Health Organization, an international commission is expected to review the measles elimination status of the United States and Mexico in November 2026. The review will examine whether continuous transmission has returned after years of outbreaks and declining immunization coverage.
The record number of measles cases is therefore more than an alarming annual statistic. The confirmed count represents only the infections that were diagnosed, investigated and officially reported. With widespread outbreaks already exceeding last year’s historic total before August, the remaining months of 2026 will test whether public health agencies can restore vaccination coverage and interrupt transmission before the country loses a major achievement that took decades to secure.
