Fiji’s Hidden Crisis: Drug Epidemic Sparks World’s Fastest-Growing HIV Outbreak

Shared Needles and Methamphetamine Use Drive a Dangerous Spike in Cases Among Youth and Newborns

Beyond the luxury resorts and crystal waters, the South Pacific nation of Fiji is currently grappling with the world’s most aggressive HIV outbreak.

This public health crisis is tied directly to a local methamphetamine epidemic that has transformed the way the virus spreads across the islands. Statistics show a terrifying trajectory: after recording 1,583 new infections in 2024, the first half of 2025 alone saw 1,226 new cases. In a country with a population of less than one million, health experts warn that the number of infections could double by the end of the year.

The primary driver is the shift toward injected drug use. Data from the World Health Organization indicates that roughly half of those currently in treatment for HIV have a history of injecting drugs and sharing equipment. The WHO pointed out that “new evidence highlights unsafe injecting drug use is a major driver of transmission and exposes urgent gaps in access to prevention and stigma-free care.” This trend often involves communal sessions where users share the same needle, making transmission almost inevitable.

This crisis is hitting the younger generation and the most vulnerable families the hardest. The majority of new cases are concentrated in people aged 15 to 34, and there is a rising concern over mothers passing the virus to their infants.

Sesenieli Naitala of Fiji’s Survivor Advocacy Network highlighted the gravity of the situation, mentioning a 10-year-old child who contracted the virus through a shared needle. She noted, “More young people are using drugs.” For many living in poverty, an HIV diagnosis is often overshadowed by the immediate struggle to survive. Ben Morrison of the nonprofit Inspire Pacific explained the grim reality for many youth, “HIV is like, what’s that? Okay, I got a sickness, but look at my life: I don’t have a dad, I don’t have a home, I don’t eat on a daily basis. So what’s another diagnosis from a doctor to me?”

In response to the emergency, the Fijian government is moving toward harm-reduction programs that include free sterile syringes to curb the spread. Hon. Dr. Atonio Lalabalavu, Fiji’s Minister for Health and Medical Services, stated, “The Ministry recognizes the vital role of sterile needles and syringes in preventing new infections linked to equipment sharing.”

While the National HIV Outbreak and Cluster Response Taskforce is working with global partners to roll out these programs, the challenge remains reaching marginalized communities that are often too afraid of the stigma to seek testing or help.
